injury or a sickness that occurs while you are covered under
this program. Some additional wellness benefits may be part

Your
school brochure will state the deductible, co-payments and
maximums if applicable. There are exclusions and limitations
in the policy, so you will need to refer to your school's

this insurance, must I go to the University Health Center
first? |

You are encouraged
to go to the University Health Center first whenever possible.
Many times your deductible will be waived or reduced if
referred from the Student Health Center. Please refer to
the Student Health Center or SHC Referral Requirement in
your brochure. |

I go to any doctor? |
| A. |
Yes,
however, you are strongly encouraged to go to your University
Health Center first whenever possible, and then to a physician
participating in the Preferred Provider Network (log in
and see your school's PPO contact page). This will save
you money. |

do I get a claim form? |

Log
in and you can print the appropriate claim forms from your
school site. |

How
do I file a claim? |
| A. |
Attach
your itemized bills to a completed claim form and mail to
the claims administrator's office which is located on the
claim form, the back panel of your brochure, or on your
I.D. card. |

What if I am out of state when I get sick or injured? |

This
insurance plan will cover you for any illness or injury
which occurs out of state. Simply follow the same procedures
in filing a claim. |

do I obtain an I.D. card? |

An
I.D. card will be mailed to the address on your enrollment
form. Call 1-800-922-3420 if you don't receive one within
2 weeks of enrollment or if you have lost your I.D. card. |
