to print be sure to print all pages of the information you need.
You may enroll using the application. Mail the signed application,
with a check, to the address located in the application instructions.

Please add "Student" information where
it requests "Employee"
and enter the "University" where it requests "Employer."
Pharmacy - until the pharmacy benefit is setup
in the system and eligibility is loaded, students will need
to pay for their prescriptions out of pocket; however, they
will be able to be reimburse once the system setup is complete.
Attached is the pharmacy claim form that can be provided to
the students who require reimbursement.
An 800# has not been setup yet for this customer, but should
be available sometime this week. Below is an 800# to the service
transition team (STT) that can be used by the students for assistance
regarding their benefit coverage until the assigned 800# is
available and eligibility is loaded into the UHC system.
Please direct students to use this 800# until further
notice.
